How to complete an Application to Import Plant Materials - Horticultural Products
The Application to Import Plant Materials consists of the following sections:

Importer details
Enter the name, street address, telephone number, facsimile number and email address (if applicable) of the person or organisation importing the goods into Australia. In the space provided, enter the contact person's name for this consignment. This name will be included on the Import Permit.
Lodging online:
If you are lodging your application online, you must provide an email address.
Exporter details
The exporter is the individual or organisation who is sending the commodity to Australia. Provide the exporter's street address, telephone number, facsimile number and email address (if applicable).
Where guidelines allow sourcing of the same commodity from more than one supplier in the country of origin, you may nominate 'various' as the exporter. In these cases, address details are not required for each supplier.
Product/commodity details
You must provide a thorough description of the product or products that you are importing. If you are intending to import live plants and/or seeds you are required to provide identification to both genus and species level. It is also essential that you indicate the type of material, for example, tissue cultures (with or without media), cuttings, bulbs, tubers, etc. The product name on the application form should correspond with the product name on the packaging, invoices and packing slips. Use the following examples as a guide for the detailed requirements.

* Please note: AQIS cannot issue an Import Permit until the scientific name is provided.
Proposed end use
The end use of imported products into Australia may contribute to quarantine risk. It is the responsibility of the individual importing these products to inform AQIS of the intended end use by selecting one of the intended end use categories listed below:
- Human consumption (only for consumption by humans)
- Processing (product requires further processing or is intended for processing)
- Stock feed (agricultural feed only)
- In-vivo (for use with or exposure to animals, plants or humans. Please specify target species)
- In-vitro (include samples for analysis, not for use with or exposure to animals, plants or humans)
- Seeds for sowing (imported with the intention of sowing)
- Nursery stock (live plants and plant parts including tissue cultures for propagation)
- Other (please specify)
Country of origin
Please provide the name of the country from which the product you are intending to import was grown.
Country of export
Please provide the name of the country from which the commodity is exported.
Other details
Product preparation
Please provide details of all treatments, preparation. Where applicable, please provide manufacturing processes that the finished product you are intending to import has undergone. Please provide certified scheme/accredited source information: for seeds, live plants or plant parts please indicate if the material to be imported is from an AQIS accredited or approved source or is produced under a certified scheme. Virus and disease testing procedure: please provide details of virus and disease testing procedures that the finished product you are intending to import has undergone.
If insufficient space has been provided to supply this information, please attach details.
Location grown/collected
You are required to provide details of the:
- location where the product was grown (for example county, province, state, region, country)
- registered premises (please provide premises registration number) or or the laboratory where prepared
- treatment/collection centre in the country of origin
Genetically modified status
Please indicate if the product has been genetically manipulated or contains any genetically manipulated material. If so, please specify details.
Transport details
Mode of transport
Please indicate if the plant materials you intend to import are travelling by air or sea.
Estimated date of arrival
Please indicate estimated date that the plant materials you intend to import will be arriving in Australia.
Route details to Australia
Please indicate the travel route of the plant materials, country by country, from departure until arrival into Australia. You must contact an AQIS office in the state of import prior to arrival to confirm these details.
Location details on arrival in Australia
Where will the product be inspected and/or processed and/or held in Post Entry Quarantine?
Please provide details of the location where the imported plant products will undergo further processing or will be held for further direction or will undergo further quarantine at an approved quarantine premises. Approved quarantine premises must be approved at time of import. If you intend to utilise more than one facility for the imported commodity, provide a summary of all premises to be used.
For plants, seeds or plant propagative material requiring further growth in post entry quarantine, indicate if the material is to be grown at a government post entry quarantine facility or at an approved private post entry quarantine facility. Please provide the premises registration number.
It is the importer's responsibility to ensure that space is available in the post entry quarantine facility for their material.
Seized goods
Please indicate if the application relates to a product which has been seized on arrival in Australia. If so, provide a reference number and other details that relate to the seizure.
Approved Import Permit
Please indicate your prefered mode of delivery. Note, if email is selected the terms and conditions have to be accepted.
Broker/Agent Details
Please provide Broker/Agent details including name, address and telephone number.
Manufacturing details
Please indicate if you intend to submit additional information regarding ingredients and preparation (e.g. manufacturer's declaration, process flow charts).

Lodging online:
If you are lodging your application online, additional information can be attached by pressing the 'attach' button, browsing for electronic file, selecting file and clicking 'add/replace file' button. To return to application press 'OK'. Alternatively, attachments can be emailed to the relevant AQIS area. The appropriate email address is provided in your eLodgement application form. Your email must clearly reference your record number, which will be emailed to you when your application has been submitted.
Importer Declaration
The importer declaration outlines the legal obligation of the individual that is applying for the Import Permit and must be filled in by all applicants. Please read carefully before signing the declaration.
Payment details
Please provide payment details and confirm your payment method.

If you are lodging your application online, you must pay by credit card.
