Application form
AQIS aims to approve all import permit applications within 10 business days from the date of receipt. If any section of the application is completed incorrectly or further information is required, AQIS will attempt to contact you. This may result in a longer application processing time. Please allow additional time if you have elected to receive your import permit by fax or delivered by mail.
There are two options for completing and submitting the Application to Import Disability Assistance Dogs.
1. Complete and submit electronic application form online (eLodgement)
The Application to Import Disability Assistance Dogs can now be submitted electronically within our secure environment. The application form consists of three sections:
- Importer and exporter details
- Commodity details and additional information
- Supporting documentation and importer declaration
