Application to Import Disability Assistance Dogs
There are two options for completing and submitting the Application to Import Disability Assistance Dogs.
Please note: In accordance with the Quarantine Service Fees Determination 2005, Import Permit fees do not apply for the Application to Import Disability Assistance Dogs.
Application form
1. Complete and submit electronic application form online (eLodgement)
The Application to Import Disability Assistance Dogs can now be submitted electronically within our secure environment. The application form is made up of 3 steps: Step 1 requires importer and exporter details, Step 2 requires animal and transport details, Step 3 requires supporting documentation and importer declaration.

Submitting the application form
Prior to lodging your application with AQIS, please review how to complete an Application to Import Disability Assistance Dogs to ensure all information is completed correctly.
AQIS aims to approve all import permit applications within 10 working days, provided that all required information has been received at the time of submission. The omission of any required information will result in an extended processing period. Please allow additional time for the import permit/s to be received by fax or delivered by mail.
